Moving Home Minnesota services (MHM)

Moving Home Minnesota is a case management service that is available through the state to assist individuals relocate from eligible institutions such as a Nursing Home to gain access to needed medical, social, educational, financial, housing and other services and supports that are necessary to move from an eligible institution to the community. Services available under the Moving Home Minnesota promote independent living. When people return to the community after long stays in an institution like a nursing home, they may need some support which is done through an assessment to determine a new level of care.

Services are available through MHM include:

 • Caregiver education (Family Memory Care)

 • Case consultation and collaboration (Pre-discharge and post-discharge)

 • Certified peer specialist services

 • Comprehensive community support services

 • Durable medical equipment and assistive technology

 • Environmental modifications

 • Membership fees

 • Moving Home Minnesota Case Management Services

 • Overnight Assistance

 • Non-medical transportation to find housing and/or employment

 • Personal emergency response systems

 • Psychoeducation services

 • Respite services

 • Supported employment services

 • Tools, clothing and equipment necessary for employment

 • Transition planning and transition coordination services
